Job summary

City of Alb-Early Intervention seeks a Care Management Assistant to support care management staff and assist in admissions and discharges, ensuring adherence to hospital and department policies. Qualifications

• High school diploma or GED required; vocational/technical training preferred.
• Associate degree may be considered nonessential.
• Bilingual in English and additional languages listed in the credential section is beneficial.

Responsibilities

• Assist Care Management staff in care management and admissions/discharge processes.
• Identify payment sources and complete forms (SSI/ISD) to facilitate admissions and discharges.
• Make calls to agencies to obtain funds for meals, lodging, and services.
• Enter patient information into computer systems and document care services.
• Support patient safety by following policies and reporting concerns.
• Encourage patient participation and questions about their care.
