Conviva Senior Primary Care in the United States is seeking a Care Integration Team Manager to lead nurses, care coaches and social workers in an interdisciplinary approach focused on high-needs patients. You will balance direct patient caseload with team leadership, program delivery and quality oversight.

Qualificações

• Active RN, LPN, LVN, PharmD licensure or EMT certification required.
• 5+ years nursing, case management, or disease management experience.
• 2+ years leadership experience.
• Experience with transitions of care and senior populations.
• Excellent clinical competencies across chronic conditions and meds.
• Experience in primary care value-based/managed care settings.

Responsabilidades

• Leads daily Care Integration Team operations including productivity, quality and performance management.
• Ensures program goals are met and uses dashboards/metrics to drive improvement.
• Sets clear expectations and provides regular feedback and coaching.
• Develops team capabilities through training and development sessions.
• Maintains clinical program integrity and escalates issues appropriately.
• Manages a caseload of high-risk patients and oversees care planning.
• Builds partnerships with market leaders and stakeholders to improve outcomes.
