Hill Country Mental Health and Developmental Disabilities Centers is seeking an MCOT Care Coordinator for Kerr County. You will provide crisis intervention, assessment, and recovery planning, collaborating with individuals and families to build person-centered plans and coordinate with partners for effective outcomes.

The role requires a bachelor’s degree in a human-services field and QMHP-CS credentialing within three months. On-call rotations and flexible hours are expected.

MCOT Care Coordinator (Kerr County MH Center)

Help us help others:

TheMental Health Crisis Care Coordinator with the Mental Health Mobile Crisis OutreachTeam (MCOT) will provide crisis intervention, prevention, and postventionservices to individuals experiencing a mental/behavioral health crisis in Kerr County. They will assess needs, develop treatment plans, and effectivelycollaborate with individuals and their family members. The MCOT Care Coordinatorwill assess strengths and needs and, in collaboration with the individual,develop a person-centered recovery plan. The Crisis Care Coordinator willprovide skills training, crisis prevention/management, linkage, and referral tosupport goal progress for children/adults and families living with mental healthchallenges. This compassionate, caring, and supportive team member willcontribute to the skills improvement, achievements, and overall potentialsuccess of a child/adult and their state of well-being.

You will get to:

• Assess individual needs and collaboratewith people to develop and execute effective recovery and life management plans.
• Provide relevant skills training to improvedaily life.
• Collaborate with service providers andagencies to be a liaison for effective care and positive outcomes.
• Collaborate with, and provide outreachto, community partners, organizations, and stakeholders.
• Maintain compliance with all legalrequirements and company policies and procedures.
• Consistently meet with individuals tomaintain progress as well as compliance.
• Work alongside law enforcement andemergency medical personnel

You'll definitely need:

• A bachelor's degree from an accredited college or university in psychology, sociology, behavioral or human services, or a related degree with the ability to be credentialed as a QMHP-CS.
• Preferred: At least one year of experience implementing the Needs and Strengths Assessments (CANS/ANSA) or the Texas Resilience and Recovery Model for Adults or Children (TRR guidelines).
• Maintain a valid Texas state Driver's License and auto insurance.
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.

We'll also want you to:

• Be available to manage crisis situations as they occur, sometimes in off-scheduled hours.
• Participate in an on-call rotation to respond to mentalhealth crisis situations, up to twice weekly. Weekendrotations on Sundays.
• Employ excellent verbal and written communication skillsto engage and report effectively.
• Maintain a well-organized schedule while allowing roomfor flexibility and adjustments.
• Maintain accurate and timely collaborative documentation.
• Sustain a calm demeanor when under pressure to meetdeadlines and handle sometimes difficult situations.
• Provide exemplary customer service and care toindividuals with their best interests at heart.
• Work autonomously to complete assignments within companyand state parameters with a high level of quality.
• Must become certified in PMAB, First Aid, and CPR within three months of employment and maintain certification without interruption thereafter.
• Document, maintain, analyze, and reportdata using Google Docs, Excel, and Microsoft TEAMS
