Catholic Charities Eastern Washington is seeking a supportive housing care coordinator to serve households in CCEW housing and those transitioning from Eastern State Hospital or eligible under the Medicaid Transformation Demonstration. The role focuses on care coordination with partner agencies to help clients move toward personal independence.

Qualifications

• BA in Psychology or Social Work from an accredited institution.
• Preference for lived experience with chemical dependency or mental health disorders.
• Minimum of two years’ experience in direct treatment of mentally ill persons.
• Knowledge of community services.
• Must have a valid Driver’s License and pass background check.

Responsabilités

• Provide supportive housing care coordination services for households living in CCEW housing or transferring from Eastern State Hospital, or eligible under the Medicaid Transformation Demonstration.
• Coordinate with partner agencies involved with households to move toward personal independence.
• Act as landlord liaison during housing search and coordinate tenancy processes.
• Link clients with primary care, health homes, substance abuse treatment, vocational and social supports.
• Maintain HIPAA confidentiality and follow agency policies in all duties.
