Job summary

Catholic Charities Eastern Washington is seeking a Care Coordinator for supportive housing services. The role supports households discharging from Eastern State Hospital and those in Medicaid Transformation Demonstration programs.

You’ll partner with treatment teams to move clients toward independence and community integration. Responsibilities include intake, service planning, landlord coordination, and linking clients to primary care and social supports.

Qualifications

• Education: BA in Psychology or Social Work, or equivalent from an accredited institution.
• Preference for lived experiences with chemical dependency or mental health disorders.
• Knowledge of community services and minimum two years’ direct treatment experience in mental health.
• Experience working with mental health and poverty-related issues.

Responsibilities

• Provide Supportive Housing services per professional guidelines.
• Coordinate with landlords and treatment teams during tenancy and housing search.
• Visit clients from Eastern State Hospital and complete intake; develop service plans.
• Act as care coordinator among involved parties and plan integration goals.
• Support households in applying for Housing through MTD and Amerigroup where applicable.
• Link clients to primary care, health homes, substance abuse treatment, and social supports.
• Assist with independent living skills and budgeting coaching.
• Maintain timely documentation and client records.
• Follow HIPAA privacy protections and agency policies.
