Job Summary

The Care Coordinator serves as a central point of contact for patients, providers, and clinical staff to ensure seamless coordination of care. This role is responsible for scheduling appointments, communicating treatment plans, facilitating follow‑up activities, and supporting patients throughout their care journey. The Care Coordinator promotes positive patient experiences and optimal outcomes through proactive communication, organization, and collaboration.

Key Responsibilities
  • Coordinate and schedule patient appointments, evaluations, procedures, and follow‑up visits.
  • Communicate treatment plans, appointment details, and care instructions to patients and caregivers.
  • Serve as a liaison between patients, healthcare providers, and other members of the care team to facilitate effective communication.
  • Monitor and track patient progress, ensuring recommended follow‑up care and services are completed.
  • Obtain, organize, and maintain accurate patient information and documentation within the electronic medical record system.
  • Respond to patient inquiries and address concerns in a timely, professional, and compassionate manner.
  • Coordinate referrals to specialists, community resources, and ancillary services as needed.
  • Verify that required authorizations, orders, and supporting documentation are obtained prior to scheduled services.
  • Assist patients in understanding their care plans and navigating the healthcare system.
  • Collaborate with clinical and administrative teams to identify and resolve barriers to care.
  • Maintain confidentiality and comply with all applicable privacy, regulatory, and organizational requirements.

Qualifications
  • High school diploma or equivalent required; Associate degree in healthcare administration, business, or a related field preferred.
  • 1–3 years of experience in healthcare coordination, medical office administration, patient services, or a related role preferred.
  • Knowledge of medical terminology and healthcare workflows preferred.
  • Experience using electronic medical record (EMR) systems.

Work Environment & Physical Requirements
  • This position operates in a healthcare clinic setting with frequent interaction with patients, caregivers, and healthcare professionals in a fast‑paced environment.
  • Prolonged periods of sitting and working on a computer.
  • Frequent communication with patients, providers, and staff in person and by telephone.
  • Ability to occasionally lift and move office materials weighing up to 15 pounds.
