Rising Star Home Care Services in Edgewood, MD, is seeking a Care Coordinator (Full Time). The role manages the master schedule, coordinates caregiver assignments, and ensures all client hours are covered. You will visit client homes, screen applicants, and maintain complete files to support reliable care delivery.

Ideal candidates hold an active CNA/GNA, with 1+ year in home care and strong organizational skills. Travel to client homes and LTSS familiarity are a plus.

Formación

• Active CNA or GNA certification is required.
• At least one year of hands-on home care experience.
• Process-minded with checklists and clean files.
• Follows through and takes ownership.
• Comfort with scheduling software and EVV (LTSS experience a plus).
• Warm, clear communication with caregivers, clients and families.
• Valid driver's license and reliable vehicle; travel to client homes.
• Able to pass a criminal background check.

Responsabilidades

• Build and manage the master schedule for clients and caregivers.
• Handle call outs and fill open shifts quickly.
• Visit client homes to introduce new caregivers and check in with families.
• Screen applicants and ensure complete onboarding files.
• Correct missed clock-ins/outs the same day in LTSS system.
• Track CPR, training, and certifications; renew before expiry.
• Coordinate client assessments with nursing team.
• Document processes to improve monthly operations.
