Travel: Up to 25% (varies by assignment, may include up to 100% within the NTBHA catchment area)

Shift: Day; flexible hours including evenings, weekend shifts, and occasional overnight duty.

Role Summary

North Texas Behavioral Health Authority (NTBHA) is responsible for developing a collaborative system of care for children, adolescents and adults with severe mental illness and other serious emotional disturbances, as well as those experiencing a substance use disorder. The Care Coordinator – Rockwall County provides timely, trauma‑informed, person‑centered, recovery‑and‑resilience oriented interventions to help individuals in crisis gain recovery and stability in the community.

Responsibilities
  • Work autonomously to make decisions consistent with applicable rules, regulations, policies and procedures.
  • Create, analyze, prepare, and maintain records, files, documents, reports, and correspondence.
  • Recognize problems, identify solutions, and facilitate resolutions.
  • Maintain knowledge of community resources, local behavioral health systems, coordination principles and program policies.
  • Communicate effectively with individuals from diverse cultural and socio‑economic backgrounds through oral and written communication.
  • Collaborate constructively with other professionals, staff, community representatives, persons with disabilities, and family members.
  • Foster strong, positive relationships with inter‑agency and intra‑agency team members, encouraging interaction across organizational lines.
  • Respect diverse views and contribute to a professional, tolerant, civility‑based environment.
  • Provide trauma‑informed, resilience‑oriented care with cultural humility.
  • Use a personal computer proficiently; advanced Microsoft Office Suite skills (Outlook, Word, Excel, PowerPoint) and ability to learn company‑specific programs.
  • Participate in staffing decisions to determine best treatment options; collaborate with internal departments, law enforcement, state and local hospitals, and community agencies.
  • Offer alternatives to arrest, emergency department utilization, and inpatient psychiatric care; perform screenings, assessments, crisis follow‑up, and transitional services to connect individuals with additional care and prevent re‑emerging crisis.
  • Provide skills training and case management focusing on individual risks to reduce recidivism, hospitalization, and/or homelessness.
  • Travel across NTBHA catchment counties; maintain reliable transportation, a valid driver’s license, and car insurance; accommodate flexible work hours including after‑hours cellular availability, evenings, and periodic weekend or overnight shifts.
  • Use fleet or personal vehicle for business needs, subject to management approval.
Qualifications
  • Bachelor’s degree in psychology, social work, or a related behavioral health field (Master’s degree preferred).
  • One or more years of experience with direct care of persons with serious mental illness and/or substance use disorders; preference for psychiatric crisis service settings.
  • Experience as a member of a treatment team that develops and monitors treatment plans for chronic and serious mental illness.
  • Knowledge of local community resources, providers, and social support for homeless and indigent populations within the NTBHA service area.
  • Understanding of, and experience with, HHSC programming and systems such as Texas Resilience and Recovery, CMBHS, and CARE.
  • Valid driver’s license and clear background record required for healthcare organization under state and federal contracts.
