Minnesota Urology is seeking a Full-Time Care Coordinator- Registered Nurse in Edina, MN. You will collaborate with the APC Care team to guide patients through the cancer care pathway and support policy development for prostate cancer care.

Responsibilities include coordinating treatments, managing IV access, monitoring side effects, and coordinating with specialty teams to ensure seamless communication and high-quality care.

Qualifications

• Minimum 2 years of oncology experience or cancer patient care in a clinical setting.
• Proficiency with MS Office Suite, EMR, Practice Management, and Lab Information Systems.

Responsibilities

• Coordinate patient navigation through the APC Care Pathway and act as educator and primary contact.
• Assist APC Physician/Provider Champions with policies, protocols, and materials for prostate cancer care.
• Plan and coordinate treatments across UA Division cancer programs (bladder, kidney, prostate) and administer specialty oncology treatments.
• Manage IV access and monitor medication side effects during nursing visits; coordinate consultations with specialty physicians and external teams.
• Ensure high-quality care and clear communication across the patient journey.
