Minnesota Urology, P.A. is seeking a Full-Time Care Coordinator- Registered Nurse in Edina, MN.

This onsite role focuses on guiding patients through the APC Care Pathway, advocating for individuals, and coordinating oncology care with a multidisciplinary team. You will support policies for prostate cancer care, manage IV access, and liaise with physicians and external teams to ensure timely treatment and seamless communication.

Qualifications

• Minimum 2 years' experience working with cancer patients in a clinical environment.
• Proficiency in MS Office Suite, EMR, Practice Management, and Lab Information Systems.
• Holding a valid Minnesota Registered Nurse license is required.

Responsibilities

• Coordinate patient navigation through the APC Care Pathway as a care coordinator.
• Act as advocate, educator and primary contact for patients in the APC program.
• Assist in developing and maintaining policies, protocols and materials for prostate cancer care.
• Plan and coordinate treatments across UA Division cancer programs (bladder, kidney, prostate).
• Administer specialty oncology treatments and manage IV access.
• Evaluate medication side effects during nursing visits and coordinate specialty consultations.
