CarepathRx transforms hospital pharmacy from a cost center into an active revenue generator through a powerful combination of technology, market-leading pharmacy services, and wrap-around services.
The Care Coordinator is responsible for managing the case coordination for patients assigned to them. This includes ensuring an excellent level of care regarding infusion pharmacy services.
Responsibilities
- Maintain complete, organized, and accurate files for all patients related to service coordination: case management, nursing services, pharmacy preparation and delivery, and relevant data.
- Input and update patient data from notification through verification of benefits, initiation of care, follow-up, and discharge.
- Complete all required reports timely, including daily, weekly, monthly, quarterly, and annual reports.
- Adhere strictly to policies and procedures, ensuring compliance with state guidelines and current knowledge of all relevant areas.
- Identify and resolve case coordination issues, documenting concerns and resolutions with the Intake Manager.
- Maintain professional and courteous communication with internal and external stakeholders involved in patient care, such as hospital discharge planners, physicians, and health benefit providers.
- Participate in orientation and training of new staff, including referral processes, documentation, and cross-training.
- Verify and train healthcare professionals on current practices related to home infusion care, including educational assessments and professional meetings.
- Engage in QA&I activities, monitoring outcomes and working towards program goals.
- Maintain patient confidentiality at all times.
- Participate in continuing education as requested.
- Perform other duties as assigned.
Qualifications
- High School diploma or GED.
- At least one year of healthcare, contact center, or medical experience in home health or infusion fields; two years of home infusion experience with strong clinical knowledge is preferred.
- Proficiency in Microsoft Suite.
