Your Role:

• Provide monthly calls and support to patients enrolled in the CCM program.
• Utilize your nursing skills to educate patients and their families.
• Monitor patients' progress and work with physicians to ensure quality patient care.

Qualifications:

• Active California LPN license (must be a California license, not a multi-state or compact license).
• Experience in geriatrics or primary care.
• Ability to sit and talk on the phone for long periods.
• System: You need a Windows i5 (or newer) processor, and Windows Pro (not Home). This can be a laptop or desktop.
• Residency in any of the following states: NV, NM, AZ, CO, UT, WA, OR.

The Pay:

• The California LPN pay starts at $28.85 per hour.
• The pay structure includes a base pay plus a bonus per call. On average, our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
