Job summary

SalusCare, Inc is seeking a compassionate Care Coordinator with experience in multi-campus care for patients with mental health and addiction diagnoses. Skilled in documentation, communication with treatment providers and creating a therapeutic environment for our patients.

You will link patients to needed resources, coordinate housing, social activities and other supports to promote independence and improved daily living. Collaboration with the treatment team is essential.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation or related field.
• Competence with Windows and common office software.
• Ability to work with multidisciplinary teams and document care.

Responsibilities

• Link patients to needed resources and coordinate services.
• Coordinate resources such as housing, mental health and addiction services, and social activities.
• Develop listings of community resources to expand patient options.
• Consult with treatment team providers and communicate patient needs.

Job Responsibilities

What you will do

• Successfully work with patients in an effort to link them to needed resources, coordinate such resources as necessary, and monitor implementation.
• Resources may include: Recovery oriented support systems, mental health resources, substance use services and treatment, housing, financial, vocational, connection to social activities, and other resources to promote independence and enhance daily living.
• Identify and develop listings of community-based resources in order to offer patients a wider array of choices.
• Consults with other treatment team providers on a regular basis and communicate needs of the patients.

Job Qualifications

Bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development).

Computer skills in Windows operating systems required.

Must be able to attain competence with company electronic health record.
