Job summary

The University of Colorado Anschutz Medical Campus is seeking a CareCoordinator for the PROMISE Clinic in Aurora, CO. This full-time role supports pregnant and postpartum patients by coordinating referrals, scheduling, and navigation of behavioral health services.

You will maintain data in Epic, REDCap, and Excel, collaborate with clinicians, and help with grant reporting and program improvement. A Bachelor's degree in a related field and clinical navigation experience are preferred.

Qualifications

• Bachelor's degree in Psychology, Counseling, Social Work, Public Health, or related field from an accredited institution.
• Experience with healthcare systems and patient navigation is preferred.
• Strong communication and organizational skills.

Responsibilities

• Serve as primary contact for PROMISE patients, handling referrals, scheduling, forms, resources, and access.
• Manage referrals from receipt to scheduling and connect patients with care.
• Provide PROMISE and community resource information and manage Epic scheduling tasks.
• Document encounters in accordance with program and institutional requirements.
• Maintain patient confidentiality and route questions to licensed clinicians.
• Support outreach to referral sources and track data in Epic, REDCap, Excel.
• Track patient and program metrics for grant reporting and quality improvement.
• Support daily operations and program activities.
