Qualifications

• Ability to speak Spanish desirable

Responsabilités

• Guides patients from diagnosis to survivorship with comprehensive follow-up.
• Initiates and documents patient interventions and referrals for data analysis.
• Coordinates resources and consult services to provide continuity of care.
• Documents initiatives in database for QI initiatives and reporting.
• May supervise a team of Patient Navigators via chart reviews and discussions.
• Works with security in emergencies when patients miss treatment sessions.
• Provides ongoing patient teaching based on assessed needs.
• Partners with families and care teams to provide timely, compassionate care.
• Reviews and updates policies related to services provided.
