Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator to organize and lead a youth and family team across home, school, and community settings in Ohio. The role aims to support families, reduce hospitalizations, and ensure a voice and choice in services.

The position requires strong communication, Medicaid/OhioRISE familiarity, travel as needed, and proficiency with electronic health records. A $3,000 hiring bonus is offered along with competitive benefits.

Qualifications

• Experience providing services to children and families in behavioral health or related fields.
• Knowledge of Hi-Fidelity Wraparound is preferred (Certification provided at employment).
• Two years of care coordination or similar experience preferred.
• Experience with autism spectrum disorders and developmental disabilities is preferred.
• Experience in family systems, community resources, case management, or related areas is required.

Responsibilities

• Lead and facilitate team meetings for the youth and identified team.
• Provide care coordination within the family's home, community, or other locations.
• Collaborate with families, natural supports and service providers to identify needs and services.
• Maintain documentation to access and maintain OhioRISE benefits.
• Meet quality and fidelity requirements per ISBH OhioRISE standards.
• Stay current on community resources and best practices.
• Attend trainings and supervision which may require travel.
• Complete trainings within timelines and maintain certifications.
• Maintain confidentiality and adhere to HIPAA and safety guidelines.
• Utilize EHR systems for accurate client documentation.
• Travel within assigned service area as needed.
