Integrated Services for Behavioral Health (ISBH) in Muskingum County, OH, seeks a Care Coordinator to organize team meetings, coordinate services in home, school, and community settings, and advocate for youth and families within OhioRISE.

The role requires strong organizational skills, ability to travel, and experience with Medicaid enrollment and confidentiality protections. A $3,000 hiring bonus is offered.

Competenze

• Minimum requirements include a high school diploma or equivalent; or an associate/bachelor’s degree; or a master’s degree or higher.
• Preferred experience in Hi-Fidelity Wraparound and care coordination roles with strong organization and detail.
• Experience working with youth with behavioral health needs and knowledge of family systems is preferred.

Mansioni

• Develop, lead, and facilitate team meetings for the youth and their identified team.
• Provide care coordination within the family’s home, school, or community setting.
• Collaborate with families, natural supports, and service providers to meet care coordination needs and goals.
• Maintain required documentation and HIPAA compliance; travel within the service area.
