Job summary

Integrated Services for Behavioral Health (ISBH) is a community-minded behavioral health organization serving Southeast and Central Ohio. The OhioRISE Care Coordinator arranges and leads a team to support youth at home, school, and in the community, coordinating care with families and partners.

The role emphasizes voice and choice for families, documentation, travel within the service area, and adherence to HIPAA and professional guidelines, with opportunities to grow in the Medicaid program.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Must have valid driver’s license and ability to use personal/company vehicle for home/school/community visits.
• Ability to bill Medicaid without restrictions is required.
• Excellent verbal and written communication skills are required.

Responsibilities

• Lead and coordinate child & family team meetings to support youth at home, school, and in the community.
• Provide care coordination in the youth's home, community, or preferred location.
• Travel within the assigned service area for service delivery.
• Maintain required documentation and HIPAA-compliant records; ensure compliance with ISBH policies.
