Job summary

Touching Angels Healthcare in Ellicott City is seeking a Care Coordinator II to oversee non-clinical home care operations and ensure continuous caregiver coverage for clients. You will manage a high-volume caseload and act as a Level 2 escalation point while guiding hospital transitions back home.

The role includes administrative recruiting support, on-call rotation participation, and a defined path to become Care Coordination Lead.

Qualifications

• High school diploma or GED required; Associate's degree preferred.
• Minimum two years of healthcare scheduling or care coordination experience.
• Demonstrated ability to manage a high-volume caseload and communicate effectively.

Responsibilities

• Scheduling and Client Services: manage caregiver scheduling for an active caseload and ensure coverage at all times.
• Escalation and Problem Solving: serve as Level 2 escalation, de-escalate concerns professionally.
• Client Transitions and RN Visit Coordination: monitor hospital transitions and coordinate RN visits.
• Scheduling/Staffing Support: assist with onboarding tasks and recruiter coordination.
• Team Support and Training: train/mentor Care Coordinators and participate in team initiatives.
• Documentation, Reporting, and On-Call: complete reports and participate in weekly on-call rotation.
