Overview

Pay Range: $22 - $23/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities

• This team conducts post-discharge outreach to AZCH Members post hospital or facility release.
• Goal to ensure services/appointments are in place to prevent readmission.
• Team culture is positive, collaborative, and supportive.
• Key projects include helping to improve Plan All-Cause Readmission Rates and Follow Up After Hospitalization Rates.
• This is a temp role to support the team while key leader is out of office on LOA.
• In the morning, staff receive list of members needing post-hospital call.
• The staff is required to make 1st call and set out reminder task for 2nd call if unable to contact.
• If successful contact is made, staff will complete post hospital assessment, help close any identified gaps, make referral to CM if appropriate, provide education about self-management resources, and complete appropriate documentation.
• Any follow up calls for unable to reach members will be completed and mailing of UTR letter if needed.
• Staff are assigned daily calls however also have follow up call.
• We are reassessing the volume of calls in comparison to overall reach rate and other team priorities/needs.
• This target may increase/decrease depending on identified efficiencies.
• This role is unique in that it carries no caseload and is a critical part of daily health plan operations.

Key Requirements and Technology Experience

• Key Skills: 2-4 years of related experience, in a medical or behavioral health office setting, with member/provider-facing responsibilities.
• Case Manager, Behavioral Health Case Manager, Medical Assistant, experience with Medicaid/Medicare members; health insurance experience is a plus.
• Health-related experience (Medical or Behavioral Health) and direct patient or client contact experience is required.
• Team player with strong communication skills and willingness to learn.
