Job summary

Sentara Health Plans in Norfolk, VA is seeking a Care Coordinator for a full‑time, 40‑hour week, 8am–5pm EST. The role delivers case management to patients and families, coordinating services and supporting adults with medical needs.

You will collaborate with a multidisciplinary team, help navigate environmental challenges, and assist those facing serious or terminal illnesses when appropriate. Experience in care coordination and health plan settings is preferred.

Qualifications

• Degree in Health & Human Services or related field required.
• Experience in care coordination and long-term care settings preferred.
• Ability to collaborate across disciplines and advocate for patients and families.

Responsibilities
