Job summary

Coastal Horizons Center, Inc. seeks a dedicated TASC Care Manager in Garner, NC to provide case management and service coordination for the TASC program.

The role collaborates with justice systems, healthcare providers, and community partners to help clients access treatment and recovery services. Responsibilities include psychosocial screenings, coordinating access to behavioral health and medical services, and ongoing care management for the justice-involved population.

Qualifications

• Bachelor's Degree or higher in a human services or related field.
• Preference for NC Peer Support Specialist certification and/or NCASPPB registration.
• Knowledge of recovery, harm reduction, and wellness-based concepts; lived experience is a plus.

Responsibilities

• Administer psycho-social screenings/assessments.
• Coordinate linkage to behavioral health treatment and other services.
• Provide ongoing professional care management for justice-involved individuals.
