Job summary

CareWorks Health Services is seeking a Care Coordinator to manage staffing, scheduling, recruiting, onboarding, and HR support for caregivers and clients. You’ll coordinate cross-functional teams and maintain high service levels in a fast-paced home care environment.

Qualifications

• High school diploma required; college coursework or degree preferred.
• 2+ years of scheduling, care coordination, staffing, recruiting, or HR experience; home care experience strongly preferred.
• Fluent in English; bilingual preferred: Vietnamese and English.

About The Role

Description

The Care Coordinator is the day-to-day link between clients, Caregivers, staffing, recruiting, and HR. The primary focus is keeping client schedules fully staffed, communicating changes quickly, and solving problems before they affect service. This role also supports recruiting, hiring, onboarding, orientation, employee relations, and basic HR documentation for Caregivers. Success requires strong follow-through, good judgment, comfort with difficult conversations, and the ability to shift priorities in a fast-paced home care environment.

• 95%+ staffing rate with open shifts addressed quickly and accurately.
• Client turnover maintained below 3.5% through responsive service and proactive follow-up.
• 90%+ client and caregiver satisfaction through clear communication and dependable support.
• New Caregivers move through hiring, onboarding, and orientation promptly and with complete documentation.
• Employee issues, attendance concerns, and policy violations are documented and addressed consistently.

What You'll Do

• Keep client schedules fully staffed by matching Caregivers to client needs, covering open shifts, resolving call-outs, and communicating changes quickly.
• Support clients, families, and Caregivers by responding to concerns, solving service issues, documenting activity, and following through until problems are resolved.
• Assist with recruiting and hiring, including applicant outreach, phone screening, interview coordination, onboarding, orientation, and follow-up on missing documents.
• Support employee relations by coaching Caregivers, addressing attendance and performance concerns, issuing verbal or written warnings, and escalating serious HR or compliance issues.
• Work closely with staffing, recruiting, HR, client services, and leadership to maintain strong staffing, client satisfaction, complete documentation, and smooth day-to-day operations.

Requirements

• Bilingual preferred: Vietnamese and English.
• High school diploma required; college coursework or degree preferred.
• 2+ years of scheduling, care coordination, staffing, recruiting, or HR experience; home care experience strongly preferred.
• Comfort handling high-volume phone, text, and email communication with clients, applicants, and employees.
• Ability to have clear, respectful conversations about attendance, performance, policy expectations, and corrective action.
• Extensive working knowledge of California Wage Order 15, California wage-and-hour requirements, meal/rest period rules, overtime, sick leave, leaves of absence, hiring practices, and other employment laws affecting home care employees.
• Strong working knowledge of California Home Care Services Consumer Protection Act/Home Care Services Bureau requirements, including Home Care Aide registration, Live Scan/background-clearance requirements, training, personnel documentation, and caregiver compliance.
• Strong problem-solving skills and the ability to make sound decisions when schedules change quickly.
• Excellent customer service, follow-through, organization, and time-management skills.
• Professional discretion and ability to handle employee and client information confidentially.
• Familiarity with home care services, basic medical terminology, scheduling practices, and caregiver/client matching preferred.
• Proficiency with Google Workspace and comfort learning scheduling, HRIS, applicant tracking, and communication software.
• Flexible, calm, and dependable when priorities change or urgent staffing needs arise.
