Job summary

Traditional Home Health Care in Wilkes-Barre, PA is seeking a Care Coordinator to monitor and support daily client services in accordance with state regulations and company policy. This full-time, salaried position includes an aggressive bonus plan as part of a generous benefits package.

You will coordinate services, manage scheduling, and grow your client caseload while building strong relationships with clients and caregivers in a family-owned, non-corporate atmosphere.

Qualifications

• Previous home health care or home care experience highly desirable.
• High School or GED required. Associate’s Degree preferred.
• Staffing experience preferred.
• Excellent verbal/written communication, time management and organizational skills.
• Ability to effectively multi-task.
• Effective problem solving skills.
• Strong presentation and training skills.
• Strong customer service skills.

Responsibilities

• Provide superior customer service and quality home care.
• Build lasting relationships with clients, referral sources, payers, and community organizations.
• Coordinate client services and manage scheduling issues.
• Manage and grow your client caseload.
• Develop strong, communicative relationships with the team.
• Interview, hire, and orient reliable and compassionate caregivers.
• Function as primary support for daily EVV and payroll functions.
