Job summary

Traditional Home Care in Reading, PA is seeking a Care Coordinator to monitor and support daily services in line with state regulations and company policy. This full-time, salaried role offers a generous benefits package and a lucrative bonus plan.

You will coordinate client services, build relationships with clients and caregivers, and ensure smooth EVV/payroll operations while supporting a patient-centered home care experience.

Qualifications

• Experience in home health care is highly desirable.
• High School Diploma or GED; Associate degree preferred.
• Strong communication, time management and organizational skills.
• Ability to multi-task and solve problems effectively.

Responsibilities

• Provide superior customer service and quality home care.
• Build relationships with clients, referral sources, payers, and community organizations.
• Coordinate client services and manage scheduling issues.
• Manage and grow your client caseload.
• Develop strong, communicative relationships with the team.
• Interview, hire, and orient reliable and compassionate caregivers.
• Function as primary support for daily EVV and payroll functions.
