Traditional Home Care in Dunmore, PA is seeking a Care Coordinator to monitor and support daily services for clients, ensuring compliance with state regulations and company policy. This is a full-time salaried position with weekly pay and a generous benefits package.

The Care Coordinator will build relationships with clients and caregivers, coordinate services, manage scheduling, and help grow the client caseload while maintaining exceptional service quality.

Qualifications

• High School diploma or GED required.
• Associate’s degree preferred.
• Excellent verbal and written communication, time management and organizational skills.
• Ability to effectively multi-task and manage client needs.

Responsibilities

• Provide superior customer service and coordinate client services.
• Build relationships with clients, referral sources, payers, and community organizations.
• Coordinate client services and manage scheduling.
• Manage and grow client caseload.
• Interview, hire, and orient reliable caregivers.
• Support daily EVV and payroll functions.
