Job summary

Qualifications

• Bachelor’s degree required if not independently licensed.
• Proficiency with Microsoft Office.
• Reliable vehicle and valid driving record; travel required.
• Experience in disability services preferred.

Responsibilities

• Conduct assessment activities to determine the needs and strengths of each individual.
• Develop plan that integrates all necessary policy components and updates throughout the year.
• Complete referrals for benefits and connect individuals to in-network providers and services.
• Remain current on benefits and coverage available for applicable health plans.
• Maintain regular contact with interdisciplinary team to ensure quality support.
