Yukon Kuskokwim Health Corporation in Bethel, AK seeks a Care Coordinator to support disabled and older Alaskans under state waivers. You will complete screenings, develop care plans, and ensure timely documentation and billing while upholding professional practices and teamwork.

The role requires an associate degree in a human services field, plus at least three years of relevant experience; travel across western Alaska is expected. Yup’ik/English speakers preferred but not required.

Qualifications

• Three years full time paid direct work experience within a human services capacity.
• Understanding of cross-cultural environments/differences.
• Act as a strong and positive role model with the willingness to travel throughout western Alaska locations.
• Prefer Yup’ik/English speaking but not required.

Responsabilités

• Coordinate care for Disabled and/or Older Alaskans under Alaska DSDS waivers.
• Complete screenings and develop Plans of Care/Habilitation Plans; amendments.
• Prepare timely and accurate documentation and billing for Care Coordination Services.
• Maintain ethical professional practices and positive team relationships.
