Job summary

Yukon-Kuskokwim Health Corporation (YKHC) in Bethel, Alaska seeks a Care Coordinator for Developmental Disabilities to manage care plans, coordinate services, and bill for care coordination. The role requires an associate degree with experience in human services and the ability to travel across western Alaska.

Qualifications

• Associate degree in Social Work, Human Services, Psychology, Sociology, Special Education, Nursing, or related field preferred; High school education required.
• Three years of related experience and/or training; cross-cultural experience preferred.
• Care Coordinator with DSDS/Alaska requirements; CPR/First Aid preferred or willing to obtain.

Responsibilities

• Coordinate care for Disabled and/or Older Alaskans per Alaska DSDS requirements.
• Develop Plans of Care/Habilitation Plans and amendments; ensure timely documentation and billing.
• Schedule appointments, meetings, and trainings; coordinate providers in villages as needed.
• Maintain positive team relationships and ensure accuracy of records.
