Job summary

Yukon-Kuskokwim Health Corporation (YKHC) in Bethel, Alaska seeks a Care Coordinator for Developmental Disabilities to manage care plans, coordinate services, and bill for care coordination. Qualifications

  • Associate degree in Social Work, Human Services, Psychology, Sociology, Special Education, Nursing, or related field preferred; High school education required.
  • Three years of related experience and/or training; cross-cultural experience preferred.
  • Care Coordinator with DSDS/Alaska requirements; CPR/First Aid preferred or willing to obtain.

Responsibilities

  • Coordinate care for Disabled and/or Older Alaskans per Alaska DSDS requirements.
  • Develop Plans of Care/Habilitation Plans and amendments; ensure timely documentation and billing.
  • Schedule appointments, meetings, and trainings; coordinate providers in villages as needed.
  • Maintain positive team relationships and ensure accuracy of records.

We offer a broad range of employment opportunities and the chance to make a meaningful impact on the health of people in the region.# **Position Summary**To coordinate care for Disabled and/or Older Alaskans who qualify under the State of Alaska, Division of Senior and Disability Services requirements, IDD, CCMC Choice Home and Community Based Medicaid Waivers. Responsible for completion of screenings, development of comprehensive Plans of Care/Habilitation Plans and amendments; preparation of timely and accurate documentation and billing for Care Coordination Services, maintaining ethical professional practices and procedures; and maintaining positive and supportive team relationships.# **Position Qualifications**### **Minimum Education**Associate degree in Social Work, Human Services, Psychology, Sociology, Special Education, Nursing, or a closely related human services field preferred. High school education required. Three years of related experience and/or training required. With one or more years working with the disabled populations and/or special needs population preferred.### **Minimum Experience**Three years full time paid direct work experience within a human services capacity. An understanding of cross-cultural environments/differences. Act as a strong and positive role model with the willingness to travel throughout western Alaska locations. Prefer Yup’ik/English speaking but not required.### **License, Certification, Registration**Care Coordinator will have a care coordination number from DSDS, State of Alaska. Professional providers will be expected to provide copies of licenses and certification to the Human Resources Department. Tuberculin clearance at time of employment and annually thereafter. CPR, First Aide required or willing to complete the class within 2 months.### **Equipment/Tools**Office Equipment i.e. Computer, Fax Machine, Copier, etc.### **Specialized Knowledge and Skills**Case Management and/or previous work in native programs preferred. Care Coordinator needs to be experienced with Division of Senior and Disability Services processes & regulations. Ensures that each assigned consumer has adequately trained providers to provide appropriate support services. Coordinates scheduling of appointments, meetings, and training for direct care providers in the villages when necessary. Strong computer program skills background required. Experience writing summaries, and synopsis report writing. Ability to complete and revise documents for accuracy in a timely manner.
