Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordinator to connect families with essential community resources and coordinate services for children and youth with behavioral health needs in Pickaway County, OH.

The role requires strong collaboration with child and family teams, cultural humility, and the ability to navigate community resources. A valid driver’s license and experience with high-speed internet are expected as part of an office-based, community-facing position.

Qualifications

• Experience providing services to children and families in behavioral health or related public sector fields.
• Hi-Fidelity Wraparound knowledge preferred (Certification provided at time of employment).
• Two years of experience in a coordinated care coordination role preferred.

Responsibilities

• Identify care coordination needs with families to ensure best outcomes for children, youth, and families.
• Define cultural factors affecting strengths and family dynamics to engage and plan care.
• Coordinate family-based services in home, school, and community.
• Ensure services on care plans are appropriate and meet safety and treatment needs.
