Job Title

Care Coordinator

Overview

Integrated Services for Behavioral Health (ISBH) is a community‑minded behavioral health organization serving Southeast and Central Ohio. The Care Coordinator engages and works with children, youth, and families who have significant behavioral health needs, coordinating service linkage, care planning, and support within homes, schools, and community settings.

Compensation

Pay range: $20.19–$25.03 per hour, based on experience, education, and licensure.

Essential Functions
  • Join with family to identify care coordination needs/services in line with service delivery standards and program outcomes to ensure the best outcomes for children, youth, and families.
  • Work with families to define cultural factors that influence strengths, functioning, and family interaction styles to ensure ongoing engagement and success in care planning.
  • Identify strengths of children, youth, and families for utilization in care coordination engagement and supporting healthy outcomes.
  • Coordinate family‑based services for children, youth, and families in their home, school, and community.
  • Ensure with family that services identified on care plans are the most appropriate, least restrictive, and meet the safety and treatment needs of the child, youth, and family.
  • Engage and build positive relationships with children, youth, and families in coordination with child and family teams to support the successful integration of team members and care plans.
  • Develop collaborative and creative partnerships with community resources to meet the diverse needs of youth and families.
  • Maintain necessary documentation, participate in program evaluation, attend team and program planning meetings, cross‑systems training, and acquire knowledge of community resources.
  • Remain current with all training requirements, including High Fidelity Wraparound, Motivational Interviewing, Cultural Humility, etc.
  • Perform all other duties as assigned.
Minimum Requirements
  • Experience providing services and/or support to children and families in behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector field:
    • Three years with a high school diploma or equivalent; or
    • Two years with an associate degree or bachelor’s degree; or
    • One year with a master’s degree or higher.
  • Knowledge and experience in Hi‑Fidelity Wraparound preferred (certification provided at time of employment).
  • Two years of experience in a coordinated supportive services or care coordination role preferred.
  • Experience working with individuals with autism spectrum disorders and developmental disabilities preferred.
  • Experience in one or more of the following areas:
    • Family systems
    • Community systems and resources
    • Case management
    • Child and family counseling or therapy
    • Child protection
    • Child development
    • Strong cultural humility or responsiveness with training and experience to manage complex cases
    • Ability to work with children and families experiencing serious emotional disturbance, trauma, co‑occurring behavioral health disorders, and who are engaged with one or more child‑serving systems.
    • Excellent organizational skills with the ability to stay focused and prioritize multiple tasks.
    • Demonstrated high degree of cultural awareness.
    • Experience with multi‑need individuals and families.
    • Broad knowledge of community service systems.
    • Willingness to participate in and lead cross‑systems care coordination.
    • Effective verbal and written communication skills.
    • Ability to operate in an internet‑based automated office environment.
    • Valid driver’s license required.
    • Ability to conduct video calls and meetings in an automated office environment is required.
    • Minimum 100 Mbps high‑speed internet connection required.
