Job summary

Integrated Services for Behavioral Health (ISBH) seeks a Care Coordinator to link families to services, work with children, youth, and their caregivers in Pickaway County, OH, and coordinate care across home, school, and community.

You'll develop partnerships with community resources, maintain documentation, and participate in program planning while meeting training requirements and ensuring voice and choice in services. Salary includes a $3,000 hiring bonus.

Qualifications

• Experience providing services and/or support to children and families in behavioral health or related public sector human services.
• Must be able to work with diverse communities and collaborate with child and family teams.
• Strong organizational and communication skills required.
• Able to operate in an Internet-based, automated office environment.

Responsibilities

• Joins with family to identify care coordination needs and services.
• Works with families to define cultural factors influencing care planning.
• Identifies strengths of children, youth, and families for care coordination.
• Coordinates family-based services in home, school, and community.
• Ensures services on care plans are appropriate, least restrictive, and meet needs.
• Engages and builds relationships with families to support care plans.
• Develops partnerships with community resources to meet diverse needs.
• Maintains documentation and participates in program planning meetings.
