Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator in Pickaway County, OH. The role focuses on service linkage for children, youth, and families with behavioral health needs, coordinating care across home, school, and community settings.

The position offers a $3,000 hiring bonus and a competitive hourly pay range based on experience and licensure. A valid driver's license and strong collaboration with community resources are required.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector human services.
• Certification in Hi-Fidelity Wraparound preferred (provided at time of employment).
• Two years of experience in a coordinated supportive services or care coordination role preferred.

Responsibilities

• Identify care coordination needs with families and link to services per program outcomes.
• Define cultural factors influencing strengths and family dynamics to ensure engagement and success in care planning.
• Identify strengths of children, youth, and families for utilization in care coordination.
• Coordinate family-based services in home, school, and community.
• Ensure services on care plans are appropriate, least restrictive, and meet safety and treatment needs.
• Engage families and teams to support integration of care plans.
• Develop partnerships with community resources to meet diverse needs.
• Maintain documentation and participate in program evaluation and planning activities.
• Remain current with training requirements (e.g., High Fidelity Wraparound, MI, Cultural Humility).
• All other duties as assigned.
