Qualifications

• Experience in care coordination or case management preferred.
• Experience in coaching or motivational interviewing preferred.
• MSW or Texas licensure preferred.

Responsibilities

• Conduct client needs assessments and develop individualized Care Plans.
• Connect clients with community resources and government assistance programs.
• Provide ongoing encouragement, coaching, and care coordination.
• Conduct follow-up calls, home and hospital visits.
• Maintain accurate client documentation using Salesforce.
• Lead and support client groups.
• Collaborate with clinic staff to ensure excellent client care.
