Qualifications

• Ability to travel locally with some exposure to inclement weather.

Responsibilities

• Schedules and documents client schedules/changes/incidents in a timely manner.
• Coordinates care with management and client care team.
• Supervises and manages assigned care team, including annual onsite visit and caregiver evaluations.
• Completes Annual Client Supervisory Visits.
• Performs client file audits and rectifies discrepancies promptly.
• Communicates effectively with clients, families, community liaisons, staff, and health professionals.
• Responds to agency and client needs professionally.
• Demonstrates commitment and competency and participates in ongoing education.
