Job summary

The Source in Austin, TX is seeking a compassionate Case Manager (Care Coordinator) to support women and families with coordinated care and access to community resources. This full-time, non-exempt role requires strong organization, excellent communication, and a heart for service.

You'll conduct needs assessments, develop Care Plans, connect clients to government assistance programs, provide ongoing coaching, and maintain accurate documentation in Salesforce, collaborating closely with clinic

Required Qualifications

• Committed Christian with a personal relationship with Jesus Christ
• Passion for serving women, children, and families while upholding the sanctity of human life
• Bachelor's degree in a related social services field, Registered Nurse (RN), or Certified Community Health Worker
• Strong organizational, communication, and computer skills
• Ability to manage confidential information with professionalism

Preferred Qualifications

• Case management or care coordination experience
• Experience in coaching, mentoring, or motivational interviewing
• Master's Degree in Social Work (Texas Licensed)
