CRF Behavioral Healthcare is seeking a Care Coordinator in the San Diego area. This temporary full-time role supports adults in outpatient programs by coordinating care, linking clients to community resources, and liaising with providers to improve access to services.

You will conduct case management, help clients meet service plan goals, and engage with disengaged clients while maintaining compliance and quality of care. Bachelor’s degree required and at least one year of related experience.

Formación

• Bachelor's degree in a mental health-related field.
• At least one year of related experience and/or training.
• Must pass criminal/background clearance.
• Ability to work in a fast-paced, collaborative environment.

Responsabilidades

• Work with providers to facilitate access to program services for target population.
• Conduct case management, linkages and referrals to needed community supports.
• Act as a liaison between clinic and community partners for referrals and coordination of care.
• Assist clients in meeting service plan goals and identifying steps to reach them.
• Engage and follow up with disengaged clients.
