AME Home Care LLC is a Catonsville, Maryland-based home care provider seeking a dedicated care coordinator. You will supervise aides, schedule shifts, and maintain client and staff records, while supporting nurses and ensuring timely communication.

The role requires strong interpersonal and organizational skills, attention to detail, and the ability to thrive in a fast-paced environment. This is an in-person, office-based position with regular weekday hours.

Qualifications

• Minimum 1 year experience as a care coordinator in personal care or home health care.

Responsibilities

• Supervise, instruct, and motivate home care aides and fill shifts.
• Build strong one-on-one relationships with clients and caregivers.
• Document all information in the contact management system.
• Maintain staff rosters, qualifications, and availability.
• Support nurses in-house and in the field.
• Monitor telephony to ensure caregivers arrive as scheduled.
• Enter missing times/shifts promptly.
• Participate in staff meetings and quality activities.
• Relay patient information to staff, RN, and Manager.
• Maintain professionalism when communicating with clients.
