Crossroads Treatment Centers in Jacksboro, TN is seeking a Care Coordinator to meet face-to-face with patients, facilitate recovery services, and ensure activities are accurately documented in each patient’s record.

You will work with collateral contacts and local referral agencies to provide services not offered by the program, and participate in monthly care coordination meetings to engage patients with the right services.

Qualificações

• Bachelor’s degree in Social Work, Psychology, Psychiatric Rehabilitation, Sociology, Counseling, Vocational Rehabilitation, or Human Services Counseling with at least one year of related experience.
• RN licensure with one year of direct experience with substance use disorders or cooccurring disorders and mental illness.
• Board of Counseling CSAC or CSAC-Assistant certification under supervision as defined by regulations.

Responsabilidades

• Meet face-to-face with patients to facilitate recovery services and document activities.
• Utilize collateral contacts and referral agencies to provide services not offered by the program.
• Conduct monthly Care Coordination meetings and document outcomes to keep patients engaged.
• Collaborate with buprenorphine-naltrexone prescribing practitioner to support patients.
