Gaudenzia, Inc. is seeking dedicated professionals to support individuals on the path to recovery with case management, discharge planning, and coordinated care for those transitioning from residential substance use disorder treatment.

A minimum Associate's Degree is required; Bachelor's degree is preferred and a CAC certification is valued. The role emphasizes collaboration, documentation, and connecting clients with community resources for successful aftercare.

Formación

• Education: A minimum of an Associate's Degree (Bachelors preferred) in a Human Service field or full certification as an Addictions Counselor
• Experience: Five (5) years with Associate's Degree; Three (3) years with Bachelor's; Two (2) years with Master's
• CAC certification: Five (5) years of CAC experience

Responsabilidades

• Providing case management services, discharge planning and care coordination for individuals transitioning from an ASAM Level Residential SUD program
• Handle a caseload to aid clients in adjusting to barriers by connecting them with community resources; duties include intake services, reports, treatment and discharge plans, and records in EHR; coordinate groups and generate referrals and aftercare plans
