Job summary

Total Care Connections in Tempe, AZ is seeking a dedicated Care Coordinator for a full-time, on-site role Monday–Friday. You will be a trusted advocate ensuring safe, compassionate care for clients and effective support for their caregivers.

Responsibilities include in-home assessments, developing care plans, documenting thoroughly, and collaborating with Scheduling, Recruiting, HR, and Operations to keep the care team aligned and focused on excellent outcomes.

Qualifications

• 1 year of experience in care coordination or related role.
• Experience in home care or community-based healthcare.
• Strong communication and customer service skills.
• Excellent organization and attention to detail.
• Ability to manage multiple priorities while staying calm and solutions-focused.
• Reliable transportation with a clean driving record.

Responsibilities

• Maintain accurate, timely documentation.

• Identify opportunities to improve client and caregiver experience and contribute to quality improvement.

What You'll Do

As a Care Coordinator, you'll help ensure every client receives safe, consistent, and compassionate care while supporting our caregivers to succeed.

• Conduct in-home client assessments and supervisory visits, ensuring care is being delivered safely and according to the client's needs.
• Develop, implement, and regularly update individualized care plans.
• Build genuine, trusting relationships with clients, families, caregivers, and referral partners.
• Listen to clients and families, understand their concerns, and take ownership of finding solutions.
• Support caregiver onboarding, training, and ongoing supervision to ensure caregivers are equipped to provide exceptional care.
• Monitor the quality and consistency of care and proactively address concerns before they become larger issues.
• Ensure compliance with company policies, Arizona regulations, and medical insurance requirements.
• Maintain accurate, timely, and thorough documentation.
• Partner closely with Scheduling, Recruiting, HR, and Operations to ensure the entire care team is aligned and supported.
• Identify opportunities to improve the client and caregiver experience and contribute to continuous quality improvement.
• Represent TCC with professionalism, compassion, and integrity in every interaction.

What We're Looking For

• At least 1 year of experience in care coordination, care management, care supervision, or a related role.
• Previous experience in home care, caregiving, or community-based healthcare.
• A genuine passion for helping people and improving the lives of clients and families.
• Strong communication, relationship-building, and customer service skills.
• Excellent organization and attention to detail.
• The ability to manage multiple priorities while remaining calm, responsive, and solutions-focused.
• Reliable transportation and a clean driving record.
