Valley of the Sun Homecare is seeking a Care Coordinator to manage client care plans, oversee visits, and coordinate with caregivers. The role emphasizes ensuring service quality and timely responses to changes in condition.

The Care Coordinator serves as the primary point of contact for clients, family members, and caregivers regarding care delivery, service needs, and changes in condition. This role is responsible for overseeing client care, ensuring service quality, and maintaining accurate and up-to-date care plans.
The Care Coordinator plays a critical role in client satisfaction, caregiver alignment, and continuity of care through both in-office coordination and in-person visits.
Responsibilities include, but are not limited to:
Conduct introductory, supervisory, and ongoing client visits, including 30-, 60-, 90-day, and quarterly visits
Assess and monitor client condition, service quality, and care outcomes
Develop, implement, and update individualized care plans
Respond to and follow up on incidents, hospitalizations, and changes in condition within 24 business hours
Collaborate with Scheduling Coordinators to ensure appropriate caregiver-client matching and scheduling alignment
Ensure caregivers are properly trained and prepared to meet client‑specific care needs
Communicate and escalating caregivers performance or client concerns to appropriate leadership in a timely manner
Maintain accurate and timely documentation in the company’s home care software system
Provide consistent, high‑quality customer service to clients, families, caregivers, and referral partners
Assist with new caregiver orientation and onboarding
Participate in and support ongoing caregiver training and in‑services
Provide coaching and feedback to caregivers to support performance and client satisfaction
Maintain and strengthen professional relationships with referral sources
Support the Business Development team with community outreach and events, including health fairs and senior expos
Assist with client assessments as needed
Occasionally assist with direct care shifts during staffing shortages or high‑demand periods
Approximately 60% office‑based and 40% field‑based responsibilities
Standard schedule: Monday through Friday, 8:00 AM – 5:00 PM
Flexibility required to address client or operational needs outside of standard hours
Participation in on‑call rotation, including evenings and weekends
High school diploma or GED required; college degree preferred
Minimum 2 years of care coordination or related healthcare experience preferred
Experience in home care, home health, assisted living, or similar setting strongly preferred
Clinical background (e.g., CNA, LPN) preferred but not required
Current CPR and First Aid certification
Valid driver’s license, reliable vehicle, and current auto insurance
Ability to travel throughout the service area during the workday
Successful completion of background check and motor vehicle report
Current negative TB test or chest x‑ray
Ability to safely perform job duties in both office and client home environments
Ability to travel frequently within the service area
Ability to remain professional and effective in fast‑paced and high‑pressure situations
Strong organizational and time management skills
Excellent communication and interpersonal abilities
Ability to build rapport with seniors and families
Proficiency in Microsoft Office, Outlook, and web‑based systems
Ability to prioritize tasks, manage multiple responsibilities, and meet deadlines
Demonstrated ability to supervise, train, and support caregivers
Full‑time, salaried position with annual pay range between $55-60,000 with quarterly bonus opportunity and full benefits. Schedule: Monday through Friday, with flexibility as required to fulfill job responsibilities.
