Job summary

Green Key Resources seeks a Care Manager to serve as the primary liaison between members and the Plan, coordinating and reviewing person-centered service plans. The role focuses on health education, ongoing member communication, and ensuring a positive member experience with thorough documentation.

Required background in health care or long-term care, RN or MSW, and a bilingual preference. The position offers a hybrid schedule with 3 days in office and 2 days remote.

Qualifications

• Background in health care and/or long-term care.
• RN license or MSW degree.
• Bilingual preferred (Spanish, Chinese, Arabic, Hindi, Bengali).
• Hybrid schedule (3 days in office, 2 days remote).
• Strong communication with members across ages and socioeconomic levels.

Responsibilities

• Ongoing communication with members, monthly at minimum.
• Follow up and respond to items requiring attention, including transition of care.
• Provide health education to members and work to improve health and minimize hospitalization.
• Coordinate member care with members, family, and vendor agencies.
• Provide excellent member experience and follow up on member concerns.
• Timely and thorough documentation in members' charts.
• Participate in team meetings and supervision sessions.
• Other duties as identified as necessary parts of the role.
