Job summary

Care Manager - Care Coordination (1216 Bay Street as anchor, Multiple locations)

The Care Manager Specialist has overall responsibility and accountability for coordinating all aspects of assigned heath home member's care. The Care Manager will track and arrange appointments, educate members and coordinate other aspects of the member's health and community services. Responsibilities include but are not limited to:

• assisting members with consent forms
• conducting outreach and engagement activities
• conducting initial and periodic needs assessments
• assisting with the development and execution of care plans
• providing links to services and communicating among care management team, providers, members and families caregivers

Daytime / Afternoon

Full-time

Salary $34.314 per hour
