Job Purpose:The Care Coordinator at LIFESTREAM BEHAVIORAL CENTER is responsible for ensuring seamless care delivery to clients by coordinating various aspects of their treatment plans. This role involves working collaboratively with healthcare providers, clients, and their families to facilitate access to necessary services and resources, ultimately enhancing the quality of care and improving client outcomes.Key Responsibilities:Develop and implement individualized care plans in collaboration with healthcare providers and clients.Coordinate and schedule appointments, follow-ups, and necessary treatments for clients.Serve as a liaison between clients, families, and healthcare professionals to ensure clear communication and understanding of treatment plans.Monitor client progress and adjust care plans as needed to address changing needs and circumstances.Facilitate access to community resources and support services to aid in the client's recovery and well-being.Maintain accurate and up-to-date documentation of client interactions, care plans, and progress notes.Advocate for clients' needs and preferences within the healthcare system to ensure they receive appropriate and timely care.Participate in multidisciplinary team meetings to discuss client cases and develop comprehensive care strategies.Educate clients and their families about the treatment process, available resources, and self-care strategies.Ensure compliance with all relevant regulations, policies, and procedures in the delivery of care services.Required Education:Bachelor's degree in Social Work, Psychology, Nursing, or a related field.Required Experience:Minimum of 2 years of experience in a healthcare or behavioral health setting.Demonstrated experience in case management or care coordination.Experience working with diverse populations, including individuals with mental health and substance use disorders.Required Skills and Abilities:Strong understanding of behavioral health principles and practices.Excellent communication and interpersonal skills, with the ability to build rapport with clients and their families.Proficiency in using electronic health records and other healthcare management software.Ability to work collaboratively within a multidisciplinary team.Strong organizational skills with the ability to manage multiple cases effectively.Knowledge of community resources and the ability to connect clients to appropriate services.Ability to maintain confidentiality and adhere to ethical standards in all interactions.LifeStream BenefitsHealth/Dental/Vision InsuranceShort Term DisabilityPension Plan403(b)PTO (Over 4 weeks your 1st year!)Flexible Work SchedulesTuition Reimbursement ProgramFree Telehealth ServicesAnd More!Important NoticeAs part of our hiring process and in compliance with Section 435.04, Florida Statutes, certain positions require a Level 2 background screening.
