Electronic Caregiver in Las Cruces, NM seeks a licensed RN to join our on-site Care Coordination team. The role focuses on telephonic and in-person triage, care planning, and patient education to support safe, timely, and patient-centered care.
You will collaborate with physicians, health coaches, and the care team to manage behavioral health and emergency/critical care needs, ensuring quality outcomes and high patient satisfaction.
Work Location: This is a full-time, on-site position based at Electronic Caregiver's corporate headquarters in Las Cruces, New Mexico. This position is not eligible for remote or hybrid work. Regular in-person attendance is required.
We are seeking a Registered Nurse (RN) with behavioral health and emergency or critical care experience to provide compassionate, patient-centered care. The ideal candidate will have experience assessing and managing patients with behavioral and mental health conditions and possess the clinical judgment necessary to triage patient concerns and respond effectively to escalated clinical situations in a telephonic care environment.
The Care Coordinator is a licensed clinical professional who coordinates, facilitates, and supports patient care across the continuum through clinical triage, care coordination, patient education, and proactive patient engagement. Working within the scope of their professional licensure (RN or LPN), organizational policies, evidence-based protocols, and applicable regulatory requirements, the Care Coordinator serves as a central point of contact for patients, caregivers, providers, and members of the interdisciplinary care team to promote safe, timely, efficient, and patient-centered care.
Through telephonic and virtual interactions, the Care Coordinator receives patient concerns, assesses available clinical information, prioritizes patient needs using established protocols, and coordinates appropriate interventions. Registered Nurses (RNs) may independently perform comprehensive nursing assessments, develop nursing care plans, and exercise clinical judgment within the RN scope of practice. Licensed Practical Nurses (LPNs) provide nursing care and coordination within the LPN scope of practice, follow established clinical protocols, and elevate complex, unstable, or out-of-protocol situations to the appropriate RN or licensed provider.
The Care Coordinator collaborates with physicians, advanced practice providers, nurses, Health Coaches, and other members of the interdisciplinary care team to support care transitions, chronic disease management, patient education, and continuity of care while contributing to quality outcomes and patient satisfaction.
