Curana Health is seeking a Behavioral Health Care Coordinator to support our behavioral health programs in long-term care, senior living, and post-acute settings. This remote, high-volume role requires proactive care coordination, managing a defined patient caseload, ongoing engagement, and collaboration with interdisciplinary teams.
You will reinforce treatment plans, identify changes in patient condition, and maintain timely documentation in EHR, while upholding privacy standards.

This roleis responsible foran managing a defined patient caseload, ensuring regular patient engagement, coordinating care across interdisciplinary teams, and supporting providers in delivering high-quality behavioral health services. The Care Manager plays a key role inidentifyingchanges in patient condition, reinforcing treatment plans, andfacilitatingcommunication between patients, families, and care teams.
This is a remote, high-volume,metrics-driven role requiring strong organization, communication, and the ability to work independently while adhering toestablishedworkflows and documentation standards.
