Qualifications

Summary

The Behavioral Health Care Coordinator supports Curana Health’s behavioral health programs by providing proactive, non-clinical care coordination for patients in long-term care, senior living, and post-acute settings.

This roleis responsible foran managing a defined patient caseload, ensuring regular patient engagement, coordinating care across interdisciplinary teams, and supporting providers in delivering high-quality behavioral health services. The Care Manager plays a key role inidentifyingchanges in patient condition, reinforcing treatment plans, andfacilitatingcommunication between patients, families, and care teams.

This is a remote, high-volume,metrics-driven role requiring strong organization, communication, and the ability to work independently while adhering toestablishedworkflows and documentation standards.

Essential Duties & Responsibilities

PatientCare Coordination

• Manage an assigned caseload of patients requiring behavioral health support.
• Conduct routine patient outreach and follow-ups tomonitorstatus and needs.
• Reinforce care and treatment plans, treatment adherence, and patient engagement.
• Identifyand elevate changes in condition, safety concerns, or barriers to care.
• Communicate with patients, families, and caregivers asappropriate.

Care Team Collaboration

• Partner with interdisciplinary teams including providers, nursing staff, social services, and facility leadership.
• Support coordination of behavioral health services, including scheduling and follow-up care.
• Facilitate communication to ensure continuity and alignment of care plans.

Documentation & Compliance

• Accurately document all patient interactions and coordination activities in theappropriate systems.
• Maintaintimelyand complete records to support program requirements and compliance standards.
• Adhere to organizational policies, privacy standards, and regulatory requirements.

Qualifications

RequiredSkills

• Exhibits knowledge of pathophysiology and accepted treatment protocols for commonbehavioralhealth diagnoses.
• Ability to analyze patient records to identify gaps in care andreportto the provider.
• Ability to work in a remote environment that is free of distractions.
• Proficient computer skills and ability to adapt to various technology platforms.
• Excellent writtenand verbalcommunication skills.
• Demonstrated experience in the usage of clinical data to guide decision making.
• Must have the ability to function independently and as a member of the interdisciplinary care team.

Required Education and Experience

• Must hold an active, unrestricted compact LPNor RNor MA license.
• Ability to obtainadditionalstate licenses, as needed
• 2+ years of experiencein healthcarerequired. Care settings may include inpatient, outpatient, orskilled nursing facilities.

Preferred Education and Experience

• Case Management experience
• CCM certification(stronglypreferred)
• Experience working with Electronic Health Records
