Job summary

The Judge Group in Houston, TX is seeking an RN Care Coordinator for a full-time onsite role in an outpatient clinical setting. The position focuses on managing patient care plans, triaging clinical needs, and ensuring clear communication across the care team.

Qualifications

  • 3–5+ years RN experience in primary care or family medicine.
  • Strong patient education and care coordination abilities.
  • Experience using Epic EMR for documentation and orders.
  • Ability to manage high patient volumes and complex cases.
  • Excellent communication with patients and care teams.
  • Capable of triaging clinical needs via multiple channels.
  • Onsite, outpatient setting with fast-paced environment.
  • Active RN license required.
  • Team-based collaboration across disciplines.

Responsibilities

  • Coordinate patient care plans across providers, departments, and services.
  • Perform clinical triage by phone, EMR, or in person.
  • Review labs, imaging, and clinical results; escalate as needed.
  • Prepare patients for procedures and verify pre-op requirements.
  • Educate patients on treatment plans, medications, and follow-ups.
  • Manage referrals and authorizations in outpatient settings.
  • Document all clinical interactions accurately in EMR.
  • Serve as primary contact between patients, families, and care team.
  • Monitor patient progress and address care gaps.

RN Care Coordinator – Full-Time


Schedule: Monday–Friday, 8:00 AM–5:00 PM Role Type: Onsite, outpatient clinical setting


About The Role

The RN Care Coordinator plays a key role in managing patient care plans, triaging clinical needs, and ensuring seamless communication across the care team. This position requires strong clinical judgment, excellent patient‑education skills, and the ability to coordinate complex care in a fast‑paced environment.

  • Care Plan Coordination — Coordinate patient care plans across providers, departments, and services.
