Job summary

Mental Health Association of Rochester is seeking a Full-time Care Coordinator to support children aged 0–3 within an Early Intervention program. You will partner with families, service providers, and community resources to promote child development and family-centered planning.

Responsibilities include coordinating services, conducting assessments, leading family-centered team meetings, and ensuring accurate documentation while traveling in diverse settings.

Qualifications

  • Bachelor's degree in Social Work, Child Development, or related human services field.
  • Preferred experience working with families with special needs.
  • Valid driver’s license and reliable transportation.
  • Strong organizational, communication, and documentation skills.
  • Ability to work in various environments and handle crisis situations.
